I give permission to the publication of my work in the DVD produced after the contest or in other possible CD-ROMs or DVDs produced by Modena Province or by other publishers (by previous agreement with Modena Province) in order to promote aniMOweb. This authorisation is signed by the participant and by the author of music as well.
Modena Province is allowed to deal with the personal data in this form exclusively for the contest activities. The treatment of private data will occur according to the current privacy law (law 975/1996).
This application form must be filled in all its parts and sent via e-mail in pdf or Word for Windows formats. The information required in this form can also be enclosed in the e-mail message with the entry files. If the participant is applying for more than one section with different works (A - intro, B - games, C - short films), or for only one section with different works (C- short films) an application form for each work must be submitted.
